In accordance with Texas House Bill 4224 and Texas Health & Safety Code §181.105, the information below explains how to request records, contact BHEC, and file a consumer complaint.

1) Request Mental Health Records

To request your records, you must submit a completed records request form.

• Request the Records Request Form from our office

• Complete and sign the form

• Return it as instructed by our staff

2) Contact BHEC

Contact the Texas Behavioral Health Executive Council (BHEC) through their official Contact Us page.

3) File a Consumer Complaint

File a consumer complaint through the Texas Office of the Attorney General’s Consumer Protection webpage.
